Full Job Description

assist in the overall delivery of care in a care coordination role to patients within the mental health team at HMP Risley and HMP Thorncross You will be responsible for the assessment of care needs and the development, implementation, and evaluation of programmes of care for mental health. This includes respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
The successful applicant is required to demonstrate a "can-do" attitude, be resilient and possess excellent organisational skill. An ability to work well as part of a multidisciplinary team is essential.
Healthcare services operate 7 days a week, and the successful applicants would need to be flexible in their approach to meeting the demands of the service.
The successful candidate will work towards providing the highest standard of care for the prison population ensuring consistent and effective nursing practice as well as operational activity.
The days within the prison service are varied and there is a wide range of training and development opportunities available., Be a full member of the multi-disciplinary team (MDT).
Effectively manage own caseload accepting referrals from agreed.

Undertake formal mental health and social care assessments of new.
