Full Job Description

We are looking for an experienced and skilled Mental Health professional to join our established Home Treatment Team based in Ivybridge and covering the West Devon and South Hams region. The team is established and provides a supportive and learning environment. You will receive regular professional and managerial supervision with enhanced training opportunities. This is an exciting time to join the team as we are working towards closer integration with our primary care, 3rd sector and community mental health teams as part of the Community Mental Health Framework,

  • Triaging and assessing referrals from internal and external referrers.
  • Assessing and care planning with patients experiencing high levels of distress and at risk of needing hospital admission
  • Liaising with a wide range of internal and external agencies to ensure safe care planning
  • working within the service to support close links with our primary care, 3rd sector and community mental health teams as part of the Community Mental Health Framework
  • Participating in regular MDT and team business meetings
  • Supervising and supporting students and other trainees as required
