Full Job Description
An exciting opportunity to join the Greater Manchester Inreach Assessment Center (GMAIC).
GMAIC's role is to offer gatekeeping assessments, bed management and case management to any CYP, who may require a Tier 4 Inpatient admission. We work in collaboration with our NHS inpatient providers, for any Tier 4 referral/request for an Inpatient admission.
The gatekeeping assessment involves establishing if the threshold has been met for an inpatient admission, or if alternative least restrictive options could be attempted initially. We also think about level of relational security is required and what unit will best suit the needs of the young person.
The bed management function involves daily liaison with CAMHS Lead Provider Collaborative (LPC) and Clinical Commissioning Groups (CCGs), to seek approval/funding and ensure our CYP are appropriately placed within different types of Inpatient settings. GMAIC then offer Case Management for any CYP who is placed in an inpatient bed within the Independent sector, for any CYP with a diagnosis of ASD/LD, CYP's who have had a previous number of inpatient admissions or with extremely complex needs and CYP who will be turning 18 whilst an inpatient., You will have the opportunity to make profound improvements in how crisis care is experienced by young people and those who care for them.
You will be working closely with our inpatient colleagues and have responsibility of managing the Single Point of Access for all Inpatient referrals for the GM population of 13-18 year old young people who are being considered for inpatient care.
This will involve working with CYP with a variety of mental health difficulties and diagnosis such as Psychosis, Bipolar Disorder, Emotional dysregulationSchizophrenia, Depression/anxiety, LD, ASD, ADHD, PTSD, Conduct disorder and Attachment difficulties.
You will also have the opportunity to work closely with external services such as social care, CAMHS, FCAMHS, CMHT, CEDS, EIT, HTT to ensure the correct pathways are in place, to support our CYP's discharge from hospital.
We have seen already how we have positively impacted in ensuring that our young people are being supported within local services, and with the help of our colleagues within RRT reducing the dependency on inpatient care, meaning young people are getting supported at home, in a timely manner.
