Full Job Description

The provision of detailed thorough assessments; of patients suitable for admission to Urgent Community Response. Planning discharge home or to a Rehabilitation bed and ensuring that agreement has been obtained from all involved parties, informing & involving carers where appropriate. Assessment, development, implementation, and evaluation of care programmes. Providing comprehensive & holistic assessment of patients in the community who require support to prevent unnecessary or premature admission to hospital or nursing home. Implementation, monitoring and evaluating standards & systems of care within agreed policies & guidelines for Responsive Services. Ensuring accidents, incidents or complaints are reported & recorded following agreed procedures. Complying with the provision of the Health & Safety at Work Act and fire precaution regulations. Developing programmes of care & involvement in implementation & evaluation. Assisting with the development of clinical care pathways. )
Documenting all necessary information accurately & efficiently for the benefit & safety of patients; continuity of care & professional security. To be flexible and to participate in a seven-day rota to meet the needs of the service Assists the Leadership Team in the ongoing development of Responsive Services. Sets personal objectives through supervision and personal development plans Incorporates relevant research findings into practice and taking a lead role in standard setting and quality assurance i) Deputises for the Team/Clinical Leads when required.
