Full Job Description

This post is critical to the development and delivery of effective and high standard Continuing Healthcare provision within Bristol, North Somerset and South Gloucestershire. The post holder will hold a caseload of highly complex individuals with learning disabilities and/or autism requiring bespoke case-management, and will be responsible for clinical case management, assessments and reviews of people known to the Continuing Healthcare team., o Provide a skilled and specialist comprehensive assessment of care needs for and to clients.o Act as patient advocate for complex and vulnerable client group, with a main focus on individuals with Learning Disabilities and Autism.

  • Adhere to national framework for continuing healthcare
  • o Monitor and review care for own case load.o Undertake to provide personalised care plans where appropriate with consultation from other professionals.o Ensure care is appropriately delivered.o Liaise with acute sector, community services, social services, care agencies, nursing homes and voluntary agencies as appropriate.o Work in partnership with patients and carers to enable them to make informed choices about care and treatment to promote their health and wellbeing.o To work in a collaborative manner with, Health & Social care colleagues in the co-ordination and best interest of the client.o Carry out Mental Capacity Assessments relating to care and accommodation for the caseload.o Carry out Best Interest Meetings when appropriate.o Present cases to complex case panel for funding approval to ensure that care is safe, sustainable and equitable. o Participate in the research projects, as required ensuring that any beneficial results are implemented on an agreed basis.o Ensure that robust systems for communication within the team are followed.o Maintain clear and concise and up to date records of care, in line with approved standards.
