The Short Term Assessment, Rehabilitation and Reablement Service (STARRS) are looking for an innovative and enthusiastic Senior Nurse who will bring new ideas and share their excellent working practice within this busy, friendly and supportive multidisciplinary team. STARRS provides Nursing, Occupational Therapy and Physiotherapy services which can include specialist assessment, equipment and short-term packages of care. We are energetic service, which provides a comprehensive service to enable safe, timely Early Supported Discharge from the acute hospital, thereby, reducing the length of hospital stay by continuing patient care at home. The team is operational 7 days a week between the hours of 08:00 and 20:30. Provide the highest standard of evidence-based nursing care to patients within their own home, working with agreed management care plans and protocols. Using Healthcare digital technology to monitor patients with different Specialities. Modern technology used to monitor patients remotely with approved health applications. Monitoring patients through digital dashboard for clinical parameters and follow up the appropriate clinical action as required. Working closely with other Virtual ward Specialities service and making sure that all clinical data are captured, and patients are monitored accordingly. Undertake and manage the comprehensive assessment, planning, implementation, evaluation and auditing of clinical care provision both in primary, secondary and tertiary care when appropriate. Optimise hospital bed utilisation and reduce the length of stay by ensuring the timely, safe transfer/discharges of patients to the most appropriate and agreed environment as well as preventing inappropriate hospital admission in line with the philosophy of the acute Trust and Clinical Commissioning Group (CCG). For this post, the following are essential
