Full Job Description

Our Halton Mental Health Recovery Team are looking to recruit a band 6 Senior Nurse Practitioner full time on a permanent contract. We are a supportive, and dedicated team and there will be opportunities for professional development within the role., The post holder will support the Clinical Team Manager in carrying continuing professional and managerial responsibility for their clinical area. They will also contribute to maintenance of the overall standards, which will include assessment of need and delivery of care within the context of the Community Mental Health Framework.

The post holder will work within an integrated team to ensure continuity of care for service users on their journey through the acute service. The post holder will work from within the community environment but be expected to be flexible in terms of delivering care within a variety of settings.
