Full Job Description

The Community Enhanced Rehabilitation Team are looking to recruit a band 6 Senior Nurse Practitioner full time on a permanent contract. We are a supportive, well-resourced and dedicated team and there will be opportunities for professional development within the role., We are looking for a Registered Nurse, to work alongside our large multidisciplinary team. The role will be varied and may include (but not limited to) covering duty, acting as a keyworker to coordinate care and undertaking routine assessments. Full induction, training and shadowing opportunities will be provided to the right candidate.

This is an exciting opportunity to be part of a newly forming team delivery high quality specialist care, The post holder will support the Clinical Team Manager in carrying continuing professional and managerial responsibility for their clinical area. They will also contribute to maintenance of the overall standards, which will include assessment of need and delivery of care within the context of the Community Mental Health Framework.

The post holder will work within an integrated team to ensure continuity of care for service users on their journey through the acute service and into CMHT. The post holder will work from within the community environment but be expected to be flexible in terms of delivering care within a variety of settings. The team is currently based in Huyton but this may be subject to change
