Full Job Description

We are looking for experienced paramedic or nurse candidates with strong clinical skills, experience and previous community work, who are patient and team focused. The successful candidates will demonstrate that they are willing to support innovation, and are committed to service improvement in an extremely interesting, and challenging environment. Your aim is to support patients to remain at home and receive the treatment they needs, recognising risk and the need for onward referral as appropriate., The services provide care for acutely unwell patients in their own home (inclusive of residential and nursing homes), aiming to avoid unplanned admissions to hospital supporting earlier discharges from hospital and in line with the National Guidance for 2 hr Urgent Community Response guidelines. The multi-skilled team of therapists, nurses, social care and health care assistants provide urgent assessments, care and treatment for people living in Wirral. People are assessed within two hours and personalised support is in place within two days (usually 24 hours). The team can also provide urgent assessments within two hours for patients who experience any of the following:

  • Fall (in or outside of their home or place of residence)
  • Reduced mobility
  • Confusion / delirium
  • Worsening of dementia
  • Urgent catheter care
  • Diabetes deterioration
  • Urgent social crisis
  • Palliative patient in crisis
  • Urgent equipment requests
