Full Job Description

Golden Hello eligible! A Golden Hello payment of £2000 (£1000 upon commencement and a further £1000 following successful completion of a six month probation period) will be made, provided that the eligibility terms are met. We are looking for an enthusiastic and motivated Occupational Therapist to work in our inpatient rehab unit at Woking Community Hospital. This post would suit a Occupational Therapist either currently at, or working towards, Band 6 level, with an interest in developing specialist skills relating to rehabilitation and complex discharge-planning. Our Inpatient services work on the Complex Elderly Rehabilitation Ward in Woking Community Hospital, offering patient-centred care, working with individuals, and their families/carers, to help optimise mobility, function and well-being. Our service is based around a multi-disciplinary team model, working with Physiotherapists, Nurses, Rehabilitation Assistants, Adult Social Care and doctors, also liaising with other community services. This is a full-time post which will include some weekend working. Applications for part-time, Return to Practice, internal secondments and bank staff may also be considered. Note: we do not offer UKVI sponsorship, and so all applicants require a current right to work in the UK., This role involves working as part of a multi-disciplinary team on an inpatient rehabilitation ward at Woking Community Hospital. Our ward team assesses and treats patients with a wide variety of conditions, often complex. We are therefore looking for a therapist with knowledge and experience of a range of client groups including any of the following: inpatient & community rehabilitation, care of the elderly, neurology, medical, oncology and orthopaedics. The duties of the role include:

  • Undertaking comprehensive assessments of patients activities of daily living (ADL), mobility, posture, seating / wheelchair needs, skin viability, and environment, in conjunction with agreed patient outcomes, assessing patients jointly with other members of MDT.
  • Using sound clinical reasoning skills to develop individualised treatment, management and discharge plans, liaising with community services for onward referrals.
  • Working in partnership with patients and carers, negotiating goals and prioritising interventions which respect patients choice and values.
  • There will be opportunity to be involved in pathway redsign process, traige of referrals from acute settings and service development projects.
