Full Job Description
The Therapeutic intervention services (TIS) sits within the older persons care unit of mental health services within Nottinghamshire healthcare NHS foundation trust.
The team objective is to provide post diagnostic support to people of all ages diagnosed with dementia including educational group work and 1:1 occupational therapy including cognitive rehabilitation.
In addition, the service provides support to carers and loved ones via the team family peer support workers.
We work closely with the memory assessment services and the young onset dementia services and are supported by the Alzheimer's society and other agencies., To receive and triage referrals from within the older persons care unit for their appropriateness for the service
To assess and care plan individual need
To deliver group work
To maintain accurate records
To supervise junior staff
To assess and care plan 1:1 community based occupational therapy including cognitive rehabilitation and generalised support and signpost.
To deliver cognitive rehabilitation and other interventions to patients.
To support the team in the absence of the team leader
To offer support to those on the waiting list via phone calls and onward actions
Any other duties associated with being a registered clinician in the team., The occupational therapist will work closely with the other team members (made up of nurses, occupational therapists, assistant practitioners and family peer support workers) to process referrals into the team.
As an occupational therapist you will be delivering educational group work via PowerPoint presentation to patients of all ages with a recent diagnosis of dementia and you will support junior staff to do the same. This takes place across the county and you would be expected to work where needed although this particular role is predominantly (but not exclusively) based in the city and south aspect of Nottingham.
You will be responsible for assessing suitability for group, referring to the family peer support workers as required and generating appropriate care plans in addition to maintaining any risk assessments and patient feedback and outcome measures.
As an occupational therapist, you will work closely with the other occupational therapist to process referrals for 1:1 work and delivering generalised support and signposting in addition to delivering cognitive rehabilitation in which you will be trained. This is delivered in the community and the patients own homes.
You will have an excellent knowledge of safeguarding and risks around those living with dementia and their carers. You will have an understanding of older people.
As a senior clinician in the team you will take an accountable role in supporting all needs of our patient group and be prepared to take on any other tasks as required to deliver this service.
You will maintain all mandatory training and receive and facilitate clinical supervision.
You will work with the team to keep patients on the waiting list safe and well.
