Don't miss out on this fantastic opportunity to join our NNE Community Therapy Service as an Occupational Therapist working in the community. If you want to be part of a forward thinking and innovative service, then this is the opportunity you may have been looking for.
In your new role you will work closely with Physiotherapists, Assistant practitioners, support staff, district nurses, specialist nurses and social workers to provide the best possible care to service users in their own homes.
We are working towards a 7 day service - 8am to 8pm on rota basis. Self rostering is encouraged., An exciting opportunity has arisen to join our developing integrated community services providing clinical triage, rehabilitation, falls management, palliative care support and support timely discharges from our acute Trust partners.
You will work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness and enable health, wellbeing and self-care. You will be visiting patients within their own homes using a goal-orientated focus, to maximise patient's independence, safety and quality of life.
The post will involve cross site working and community visits. Part of the role will be in the South Nottinghamshire Transfer of Care HUB - on a rota basis with B6 colleagues. A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010., You will manage your own caseload in the community. As part of the integrated care team you will work closely with the rest of the MDT to provide the best possible outcomes for our service users in NNE., You will work with system partners to deliver care on Pathway 1 of Discharge to Assess and be part of Notts Transfer of Care Hub

You will have a degree/diploma in Occupational Therapy & be HCPC registered. You should possess excellent interpersonal and time management skills, be a good communicator and able to prioritise and work well as a member of a team. An induction period and supernumerary support will be given. Ideally you will have experience of working in the NHS in the UK and being part of our healthcare services.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.
