Full Job Description
Regularly review and cleanse patient tracking lists (PTLs) to ensure accuracy and resolve data discrepancies such as duplicate entries, outdated records, and incorrect statuses.Maintain accurate RTT and non-RTT patient data, supporting compliance with NHS performance targets.
Pathway Monitoring and Escalation:
Track patients on RTT and non-RTT pathways, ensuring timely progression and flagging any delays to the appropriate teams.Escalate at-risk patients to Operational Managers and clinical teams to prevent breaches of care standards or RTT targets.
Operational Support:
Work closely with Operational Managers to manage the administrative aspects of patient pathways, including scheduling diagnostics, appointments, and follow-ups.Assist in coordinating resources to reduce pathway bottlenecks and optimise clinical capacity.
RTT Compliance:
Support the identification and resolution of RTT breaches by maintaining robust patient tracking and ensuring adherence to 18-week standards.Provide regular reports on RTT and non-RTT pathway performance, highlighting trends and areas for improvement.
Non-RTT Pathway Management:
Ensure non-RTT patients are tracked appropriately, maintaining visibility on the PTL and adhering to agreed timeframes for care delivery.Collaborate with teams to create and monitor non-RTT pathways for long-term condition management and surveillance.
Main duties of the job
Data Reporting and Analysis:
Compile and present data to Operational Managers, highlighting pathway performance and areas requiring attention.Contribute to audits and reviews to improve the quality and efficiency of patient pathway processes.
Communication and Coordination:
Serve as a key liaison between clinical teams, administrative staff, and Operational Managers, ensuring seamless communication and workflow.Engage with patients, as required, to provide updates on their care pathways and address any concerns.
Support for Service Improvement:
Assist in pathway redesign initiatives and operational projects to improve RTT and non-RTT pathway management.Propose and implement strategies to streamline processes, reduce administrative burden, and enhance patient care delivery.
Training and Guidance:
Provide guidance and support to administrative and junior staff on pathway management processes and PTL maintenance.Ensure staff are familiar with RTT and non-RTT tracking protocols and standards
