About us

We are a team of therapists, nurses and non-registered clinicians who work across both sites within GHT to support the coordination of onward care planning for those patients who require some support when leaving hospital.

We are constantly adapting to the changing landscape within the community in relation to resources to support with discharge planning and as such, no two days will feel the same.

We will need you to be dynamic, engaged, happy to question things in a way that brings people with you, flexible in your thinking and organised in your daily tasks.

We need people who are naturally inquisitive, helpful, flexible and feel strongly that discharge plans should be made with our patient at the centre.
