We have an exciting opportunity for a dynamic and motivated pharmacist to be the Lead Pharmacist for the Integrated Care Division. This Division includes three community hospitals, a range of community services, including Virtual Wards.

Many of our patients require multidisciplinary interventions. The post holder will be responsible for leading the clinical pharmacy service, ensuring that the use of medicines is optimised through safe prescribing, handling and administration processes to ensure that patients get the best outcomes and experience from taking medicines., To assist in prescribing support at our Community Hospitals as a member of the patient facing team supporting local GPs, and Trust prescribers.

To assist in prescribing support for the Virtual Ward as a member of the multidisciplinary team supporting ACP's and other prescribers.

To provide clinical leadership and ward support at ward level to the ward at The County Hospital as required.

To support operational activity within the Pharmacy Dispensary including weekend and Bank Holidays and Pharmacy Emergency Duty Commitment (EDC) rota.
