Full Job Description

A challenging and exciting opportunity has arisen for an experienced senior pharmacy technician to join our proactive care pharmacy service of specialist pharmacy staff who work with this cohort of older adults to achieve best outcomes from medications. They do this by reviewing medications with individual patients/ carers and providing guidance on medicines use and management.
The post holder will be an integral member of the Sussex Community NHS Foundation Trust (SCFT) Medicines Management Team. The main duties are to:
+ To provide medicines management support to patients in their own homes within identified PCN areas.
+ To deliver medicines optimisation promoting and delivering high quality, safe and cost effective, evidence-based pharmaceutical advice to prescribers and others.
+ Undertake medication review/pre-Structured Medication Review (SMR) of patients with recommendations and GP records updated, as per scope of practice agreement.
+ Ensure that improved outcomes can be demonstrated through key performance indicators (KPI).
+ Working in partnership with and proactively developing and maintaining good working relationships with the multi-disciplinary team (MDT).
+ Supporting the development and delivery of practice quality improvement
+ Continuing professional development by attending relevant local and national courses such as PresQIPP, CPPE
+ Developing pharmacy workforce through practice supervision., + To work autonomously but alongside pharmacists to support the SMR work by doing medicines reviews/ Pre-SMR for proactive care patients, including those with complex medication needs.
+ To work within the proactive care team to maximise the use of the skill mix of pharmacists and pharmacy technicians, ensuring each role is working to the maximum of their scope to produce efficiencies in delivering the service.
+ To participate in regular clinical supervision and peer review of work to ensure consistent standards and to help develop individual pharmacy staff.
+ To report KPI to reflect your work.
+ Participate in SCFT Medicines Management team meetings where appropriate.

Our proactive care pharmacy service are core members of the Community Nursing Multi-Disciplinary Teams, working at PCN level with community nurses and proactive care coordinators, SPFT mental health nurses, GP Practice teams, Community Pharmacies, Social Services, Care agencies and others involved with patients and their medicines.
The post holder will be locality based in Coastal West Sussex , but may be required to travel to other areas covered by SCFT. A choice of main base will be available from Worthing, Littlehampton or Pulborough.
The postholder will be an Educational Supervisor (ES) or working towards these qualifications.
