Full Job Description

The post holder will be part of our multi-disciplinary Bed Based Care Team, at Caterham Dene Hospital, working on the ward to prevent unnecessary hospital admissions and facilitate timely hospital discharges for patients. Caterham Dene Hospital ward is an inpatient rehabilitation ward, 365 days a year, and the therapy team provides a seven day a week rehabilitation service. The post holder is responsible for providing a comprehensive and co-ordinated range of specialist physiotherapy therapeutic and preventative services which maintain or improve peoples health, independence, and quality of life. The post holder will apply knowledge and skills across a range of conditions and medical impairments, procedures and practices gained through practical experience and underpinned by theoretical knowledge. The Post Holder will need to have a full drivers licence and car for personal business use and be able and prepared to drive between community settings during their working day. The post
holder will participate in the weekend and bank holiday rota. The post holder is responsible for: Assessing and treating a caseload of patients in First Community Health and Care (FCHC or First Community) Community Hospital settings who may have complex presentations whilst liaising closely with the multi-disciplinary team to enable rehabilitation and safe discharge planning. Treating patients who may present with a variety of acute and chronic conditions and significant physical impairments due to illness, pain or injury, psychological issues, and post-operative rehabilitation requirements. Maintaining a high standard of clinical record keeping using our electronic clinical record keeping system. Providing a high standard of clinical work which will also include teaching and the supervision of junior staff, assistants, and students. Line management of junior staff.
