Full Job Description

This is an exciting opportunity for a motivated, flexible and enthusiastic individual for the KOTS service. There needs to be a special interest in working both in hospital and in the community. The KOTS service is an innovative service that bridges the care from hospital to home, covered over seven days and overseen by a band 7 team lead and Clinical Lead in Acute Medicine. The post holder will work closely with community partners, other Band 7's, Band 6's, Band 5's and assistants within the wider acute medicine team at Denmark Hill. This is a permanent, full-time post based at Denmark Hill, Kings College Hospital., The post holder will provide specialist assessment, and intervention to patients In the community. The post holder will manage a variety of patients with long-term conditions including dementia and delirium, falls, and chronic neurological and respiratory conditions. You will complete comprehensive assessments, treatment and early discharge plans for patients with complex health and social care.

As well as gaining valuable clinical experience in the community you will also get an opportunity to develop your managerial and supervisory skills providing supervision to a band 4. As band a band 6 there will be opportunities to be involved in service development and research.

Continuing professional development is encouraged through the appraisal system, peer working and support.

We invite applications from HCPC- registered Physiotherapists with experience in care of older people/ elderly rehabilitation at band 6 level with good insight into falls, dementia, Delirium and early discharge planning. Applicants should also have a sound all round level of experience.
