Full Job Description

The Integrated Community Response Team is a team a large multidisciplinary team made up of GPs, occupational therapists, a social worker, physiotherapists, nurses, a community matron, mental health nurse and support workers. The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission. The team provides immediate assessment, intervention, rehabilitation and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.

We are currently seeking a physiotherapist with a broad range of previous experience across acute and community settings. You will have completed all core rotations and preferably had previous experience working in accident and emergency and in treating acutely unwell clients in the community. You must have excellent assessment and sound clinical skills and be able to treat clients who present with varying medical diagnoses. You must be flexible, organised, have excellent time management skills in order to manage a case load effectively as the team priorities change frequently throughout the day.
As the service is community based, you will need to be a car driver and required to work long days and weekends on a rota basis.
