Full Job Description
An exciting opportunity has arisen for an enthusiastic and motivated physiotherapist to join our dynamic Admission Avoidance and Discharge Service (AADS) at ELFT. We are proud to be part of a successful partnership between health and social care, driving a proactive approach to care for the residents of Tower Hamlets. This role offers the unique opportunity to work at the forefront of integrated care, collaborating closely with Reablement Service and Initial Assessment Team (IAT) to deliver timely and effective physiotherapy interventions that prevent unnecessary hospital admissions. Moreover, the opportunity to work collaboratively across services, including pioneering work with the Podiatry Service to support homeless populations in Tower Hamlets.,
- To manage a defined caseload using evidence based /client centred principles to assess, plan, implement and evaluate interventions, maintaining associated records.
- To provide a high standard physiotherapy service to the specialist caseload across various sites within Tower Hamlet as the service requires focusing on admission avoidance and supporting timely hospital discharges.
- Actively participate in the Homeless Project, providing drop-in sessions and delivering health promotion activities to empower homeless individuals to improve their mobility, foot health, and general well-being.
- Engage in holistic care planning for vulnerable populations, addressing barriers to care and promoting self-management strategies.
- To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and develop and deliver an individualized treatment program.
- To provide leadership for more junior staff including supervision of students on placement, as agreed with senior staff anddeputies for the Senior Physiotherapists in their absence as required.
- To participate in CPD and develop skills and knowledge through participation in the in service training scheme.
- To undertake lone working when required.

- To be professionally and legally be accountable for all aspects of own work, including the management of patients in your care
- To undertake all aspects of clinical duties as an autonomous practitioner
- To be responsible for maintaining accurate and comprehensive patient treatment records in line with legal requirements and Health & Care Professions Council (HCPC) standards of practice
- To understand the legal framework around gaining informed consent for treatment and to implement appropriate procedures in line with trust policies
- To have a comprehensive understanding of patients' clinical presentations based upon advanced clinical knowledge
- To develop specialist knowledge of particular conditions and patient presentations specific to different diagnoses and treatments
- Foster effective communication and collaborative working relationships across multidisciplinary teams, including health, social care, and voluntary sector partners.
- Work with the team to evaluate the impact of the integrated model of care, providing feedback and recommendations for improvement.
- Support the expansion and sustainability of the Podiatry Homeless Project, ensuring its continued success in addressing health inequalities.
We are looking for a highly motivated, innovative, and adaptable physiotherapist to join our team in working with patients across a variety of clinical areas to deliver proactive care in partnership with multidisciplinary teams. Applicants must have a strong communication and collaboration skills and have experience in community care, with commitment to working across health, social care, and voluntary sectors boundaries. You will need to be a versatile, adaptable team player who is prepared to work across a 7 day rota.
