Full Job Description

We are looking for an exceptional individual to join the Transfer of Care Team at the Royal London Hospital as a Senior Physiotherapist Discharge Navigator. The Transfer of Care Team is a joint health and social care service aimed at facilitating discharges from the acute setting, based on the home first principle. The team prevents avoidable delays in patient discharges through coordination and planning in accordance with current government discharge guidelines. You will problem-solve to overcome challenges and work with the ward multidisciplinary teams, senior management and external agencies as well as advocating on behalf of your patients.

You will be based mainly on the wards as well as non-clinical areas, providing discharge advice and information on local authorities, voluntary agencies, and other help and support available to patients once discharged into the community. You will act as a discharge planning expert on the wards to ensure that people who do not require acute hospital care are discharged to their usual place of residence, rehabilitation units or residential/nursing home placements. You will use your clinical skills to screen referrals to ensure patients are discharged with appropriate therapy, health and social care support on discharge., As a senior physiotherapist specialist in this field you will supervise and line manage more junior staff, supporting them through teaching and guidance with their more complex caseload management.

You will have a sound knowledge of discharge planning and discharge processes. You will have a clinical background that will enable you to identify patients suitable for therapy and reablement interventions on discharge and screen referrals to community services. You will possess an ability to prioritise both clinical and operational workload. Your experience will support a pro-active approach in a dynamic and continuously changing environment. You will be confident in supporting other staff with discharge processes a variety of settings. You are a team player, with excellent communication skills, who would enjoy the challenges that this role will provide.

You will be able to manage conflicting priorities, be confident in challenging decisions at all levels and have experience of working in a high paced, high pressure environment. You will supervise more junior staff a will yourself be offered senior support and training to help develop you in your role.

We are a 7 day service operating from 08:00 to 18:00 including bank holidays. If successful, please note you will be expected to work weekends, bank holidays and late shifts as part of a rota pattern.
