Full Job Description

The successful candidate will be responsible for leading in the development and delivery of complex care plans for patients, alongside forming part of the nursing team in the delivery of care. You will be required to work within a multi professional framework and to support the Ward Manager and Clinical Lead with a variety of tasks, including supervisions for allocated members of the team.

The post holder will need to ensure that the service is based on an ethos of collaborative working between individual service users, carers and all other key stakeholders.

Post registration experience of working with adults with learning disabilities and/or working within an assessment and treatment unit is preferred, however not essential.

CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

  • Eastway is an Assessment & Treatment Unit for Adults with Learning Disabilities and complex needs. Admissions are to support individuals whom require a period of intensive assessment and treatment away from home.

    An opportunity has arisen for a Band 6 Senior Practitioner to join our team. You will be part of a supportive multi-disciplinary team who work closely with other community services in providing a holistic approach to care utilising the 6 Cs in practice, these being Care; Compassion; Competence; Communication; Courage and Commitment, and support the development of person centred care plans.
