Please see full job description attached. The post holder will play a key role in supporting the Personal Health Commissioning Service. Working within the remit of the National Frameworks for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service. The post holder will be required to manage a caseload of CHC Eligible individuals ensuring The National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes. The post holder will oversee a caseload of complex cases, undertake annual eligibility reviews and manage Continuing Healthcare packages of care, ensuring value for money placements which meet the needs of the individual and identify opportunities for development and improvement to assist delivery of best practice healthcare.
