If you do, then a role in the new Learning Disability and Autism Navigation Team may be for you. This new team will provide a county-wide navigation function, supporting workers to appropriately and effectively respond in times of challenge, including urgent and crisis situations, through the use of pre-invested services.
The team will develop and harness knowledge and networks across the Council, NHS and community partners, sharing their learning with colleagues. The team will work flexibly alongside the area team social work teams to help them navigate processes, access specialist services, provide professional 'peer challenge' and ensure approaches deliver 'best value'.
We are seeking a passionate and motivated Senior Practitioner to help develop this new navigation team and provide advice and support to area team managers/leaders on appropriate approaches to escalation and crisis. The post will also be responsible for the development of resources, created and/or obtained through the teams' work, which will be shared through learning and development activities with the wider department to improve knowledge and practice around learning disabilities and autism.
You must be confident in providing appropriate professional 'challenge' where you feel an alternative approach should be considered that offers better value and will have the effective use of pre-existing processes and services at the forefront of your approach to practice. Experience of or awareness of Continuing Healthcare processes would be beneficial but is not essential. There is a requirement to travel county-wide., This role currently offers hybrid working options which will be to subject service needs, there'll be an opportunity to discuss working arrangements for this position at interview.
