Do you have experience as a Band 5 or 6 Registered Childrens Nurse, or a social worker, physiotherapist or occupational therapist ideally with community experience, and experience with children or young people with complex health needs? Are you looking for an opportunity to develop your career?

You will work closely with families to support the delivery of individualised care in the community for children and young people with continuing care needs. This is an opportunity for both personal and professional development and you will be given support and training within a supportive team environment., NHS Sussex ICB is in the process of positively developing its Childrens Continuing Care (CYPCC) services and expanding our team. We are recruiting a Registered childrens nurse or allied professional to join our existing Childrens continuing care team, to work across Sussex as a Pan Sussex CYPCC team assessor.

This role as assessor requires a compassionate approach,within a Multidisciplinary team, to establish eligibility using the NHS National framework for children and young persons continuing care, and liaising with Health, Social care, Education colleagues and other professional colleagues and agencies, to oversee, with band 7 support, the delivery of child specific packages of care. Line management and support will be given from the Band 7 Nurse Specialists within the team, and from the Pan Sussex Clinical Operational Lead nurse. Some experience of complex, long-term health conditions and working in the community with children and young people is desirable.

Communication and IT skills are essential to this role. The job is home based with some visits to families homes, schools and respite or residential settings across Sussex will be required.
