Full Job Description

We are looking for a Senior Practitioner to join the Hammersmith and Fulham Outreach and Housing First Team. The team forms a significant part of the borough's complement of homelessness services. The team will continue to support its existing Housing First clients plus work to reduce the number of people rough sleeping in H&F, by providing street outreach interventions to support people to access accommodation; and ensure that the appropriate support is in place. The team is also increasing their cohort of Housing First complex needs clients, supporting additional clients to access and maintain long term accommodation.

You will:

.Work alongside the Lead Manager to manage the services performance against the contract.
.Be responsible for directly managing the staff team and overseeing casework practice of the team.
.You will have involvement in ensuring the finances of the service are well managed and that the team is compliant with all relevant policies and legislation.
.Foster a culture of high performance and continual striving to improve upon successes throughout your team.
.Develop and manage partnerships with key agencies.

Have excellent prioritising skills and be able to independently manage your workload.
.Be a proficient user of Microsoft Office applications, including databases.
.Have an interest in homelessness and a desire to support us to have a positive impact on the lives of homeless people.
.An excellent communicator able to form and maintain effective working relationships with all partners.
.Experience in directly managing staff, ensuring high quality casework and quick outcomes are achieved., Disability Confident
