Full Job Description

As a Senior Programme Manager, you will oversee the execution of various improvement initiatives and transformation projects aimed at enhancing safe and effective discharges. Your focus will be on implementing the Home First approach and integrating discharge to assess (D2A) models of care.

The post holder will work with the system partners and other agencies to deliver the system-wide agreed annual objectives under the Home First programme. The impact of this role is critical for the ICS, the post holder will lead on co-design and co-delivery of the set-out programme milestones.

The post holder will be a change leader in the system working with executives, senior leaders, managerial teams and operational colleagues building effective relationships and delivering change successfully through effective engagement and delivery.

The post holder will support equality, diversity and inclusion programmes, working with colleagues across the ICB and the wider ICS to develop supportive programmes that facilitate the delivery of the programme objectives.

The Home First programme is a system-wide programme which delivers a series of improvement initiatives and transformation projects focusing on the delivery of safe and effective discharges focusing on Home First and embedding discharge to assess (D2A) models of care.

This is a critical role with a significant impact on patient care and system efficiency.

Main duties of the job

Stakeholder Engagement: Build strong relationships with key stakeholders.

Data Analysis and Metrics: Establish clear metrics to measure the success of discharge initiatives. Collect and analyse data on discharge times, readmission rates, and patient satisfaction.

Process Mapping: Map out current discharge processes to identify bottlenecks and areas for improvement. This will help the system streamline workflows and implement best practices.

Training and Education: Develop training programmes for staff across the system to ensure they understand the new processes, procedures and models of care and their roles in the discharge process, empowering all system teams to foster a culture of ownership and accountability.

Pilot Programmes: Run pilot programmes for the new processes or care models in select areas before a wider rollout.

Continuous Improvement: Implement a continuous improvement framework to regularly review processes and outcomes. Encourage a culture of feedback where staff can share insights and suggest enhancements.

Patient-Centred Care: Ensure that all initiatives prioritise patient preferences and needs. Engaging patients and their families in the discharge planning process can lead to better outcomes and satisfaction.

Communication Strategy: Develop a clear communication plan to keep all stakeholders informed about the initiatives, progress, and any changes to processes with the aim of transparent communications fostering trust and collaboration.
