Senior Registered Nurse - Health Facilitation

We are seeking a highly motivated enthusiastic and passionate learning disability nurse to join the Adult Learning Disability Enhancing Physical Health team, as a senior registered Nurse / Health Facilitation nurse. The Team provides specialist advice and support to both people with a learning disability and to health services to reduce the health inequalities and premature mortality that people with a learning disability face., The successful candidate will work with health colleagues within the wider Learning Disability Community Teams, Local Authority colleagues and in liaison / partnership with primary and secondary care services, using a Person-Centred Approach to enable and support adults with a learning disability and their carers to promote the health needs of people with a learning disability and enable access to wider health services.
The successful candidate with be expected to support in the delivery of the additional functions of the team. This includes the implementation of National NHS England work programme's including The Enhanced health in care homes Framework, Stopping over medication of people with a learning disability and autistic people (STOMP), and the GP Enhanced Service for the Annual Health Checks. Other functions of the team include the transitions of from children's services to adults and the management of epilepsy from the epilepsy specialist nurse within the team.

We won't rest until everyone in our region has the mental health care they need, to lead their best possible life., The team is in the early stages of merging with the Durham and Darlington Health Facilitation team and will become the Physical health Team across Durham and Tess Valley (DTV).
This role is a full-time permanent position working hours Monday-Friday 9am-5pm
Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
Qualified nurses are expected to participate in the ALD nurse on call (Monday-Sunday 5pm-9am), Here at Tees, Esk and Wear Valley Foundation NHS Trust we want you to have a positive experience when you start your new role with us. To ensure you have great welcome, for all new starters joining the Trust, we offer two start dates per month on the 1st and 3rd Mondays of that Month.
