Full Job Description

We are looking for experienced staff from a health and leisure or NHS background to support our new programmes of care. If you feel passionate about helping individuals regain their confidence following a change in their abilities, then we would love to hear from you. You will be supported by experienced hospital and community therapist's and will follow guidance and or complete assessments of individuals in their own or health care setting to maximise their return of function. Full training and induction will be provided, you will receive regular 121's, be part of a new peer support programme and be changing the way we deliver NHS services. Our service currently runs 8am-8pm, 7 days a week and cover of these hours are expected. Weekends are also managed on a rota bases with a ratio of approximately 1 in 6'. The Integrated Community Teams include Occupational and Physiotherapy, Frailty services, Community Nursing and Older people's mental health.   • To co-ordinate and conduct rehabilitation programmes (including 1:1 and group classes) for patients both independently and under the guidance of the Physiotherapists. This will initially be in care home settings and individuals own homes.
  • To work closely with the physiotherapists and occupational therapists in the rehabilitation of patient with mixed physical diagnosis, recognising and immediately escalating any concerns around patient deterioration or lack of progression, referring back to physiotherapist or occupational therapist as appropriate
  • Responsible for recognising signs of a potentially unwell patient while under their care either in class or 1:1 setting
  • To be responsible for the diary management of their classes/patients
  • To be a resource for continued training to wider registrant and rehabilitation assistant workforce in the use of safe and effective exercise rehabilitation.
