Full Job Description
The Neuro Early Supported Discharge Team provides up to six-week of intensive neurorehabilitation to individuals with a Stroke or Neurological diagnosis, who have been newly discharged from hospital or are at risk of admission into hospital and have a GP within Southampton City or parts of West Hampshire. The multidisciplinary team comprises of Physiotherapy, Occupational Therapy, Speech and Language Therapy, Nursing, Clinical Psychology and Rehabilitation Assistant therapy, which is delivered in a client's own home or alternative community setting.
The Neuro Early Supported Discharge Team works closely with all services involved in the Stroke and Neuro Pathways in Southampton, including University Hospital Southampton Hyper-acute Stroke and Acute Stroke Units and the Wessex Neuro Centre, as well as sister teams within the Neuro Rehab Services- Snowdon Neuro Rehab Unit, Community Neuro Rehab Team, Vocational Rehabilitation Service, Neuro Rehab Gym, Transition Team, Psychology and Speech and Language Therapy team.
Intervention from the Early Supported Discharge Teams aims to facilitate improved independence and quality of life based upon agreed personalised rehabilitation goals.
The purpose of this position is to provide Senior Rehab Assistant intervention to a specified caseload of patients as part of a comprehensive multidisciplinary team, delivering intensive neuro-rehab intervention programmes based upon individualised patient goals.
An exciting opportunity has arisen for a enthusiastic and motivated Senior Rehabilitation Assistant to join the Neuro Early Supported Discharge Team within Hampshire & IOW Healthcare NHS Foundation Trust. A progressive Band 3-4 would also be considered depending on experience; this would be discussed at interview.
The Neuro Early Supported Discharge Team is a Multidisciplinary team providing specialist intensive rehabilitation to clients with acute stroke and other neurological conditions on discharge from hospital or as community admission avoidance. The post holder will be working alongside a team of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Nursing, Clinical Neuropsychologist and Rehabilitation Assistants working the Southampton and West Hampshire areas.
Working Pattern: Monday - Friday with the expectation for some weekend working.
A full and valid UK driving licence and access to a vehicle for work purposes is required.
