Full Job Description

The role is based out of Fairmount house in Leatherhead and covers the Borough's of Elmbridge, Epsom & Ewell and Mole Valley.
You will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. Your role will involve a variety of tasks including assessing people's needs and carrying out regular reviews.
You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.
You will have a caseload and work with individuals who need additional support or guidance in order to identify their needs or where best interest decisions need to be made on their behalf. They may need close monitoring or frequent review or interventions to minimise and manage risk. You may be involved in carrying out risk assessments and supporting safeguarding investigations, attending safeguarding meetings with the support of qualified practitioners.
Shortlisting Criteria

This role will suit someone with some professional experience in social care and who enjoys a public facing role. To be shortlisted for an interview your application will clearly evidence:

  • Substantial experience of working directly with people with social care needs, including visiting them in their own homes

  • Excellent IT skills including Microsoft Office packages and Internet

  • Access to a car and a driving licence

  • Ability to work on your own initiative and respond flexibly to changing priorities

  • Excellent verbal and written communication skills


  • Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.Reasonable adjustments where needed will be made for successful applicants who have a disability or long term health condition to enable them to fulfil the requirements of the job.

    The starting salary for this position is £ 32,512 per annum based on a 36 hour working week.
    This role is for a one year secondment opportunity (maternity cover) for a Senior Social Care Assistant within Mental Health Mid Social Care Team.
