Why Now?

We are excited to have been awarded funding from 'Q Exchange' to enable a 1 year project to explore health needs at the point of transition to adult services and beyond 18. The post holder will be the Senior Nurse Project Lead for Care Leavers transitioning to adult health services. Ensuring engagement with 'Q Community' and 'Bucks Youth Voice' to identify best ways to engage with young people to hear their ideas about support required. Working alongside BHT's Quality Improvement (QI) team reporting project progress and outcomes in accordance with 'Q Exchange' requirements.
