Full Job Description

Cornwall Partnership NHS Foundation Trust (CFT) is excited to recruit for Senior Staff Nurse within Camborne and Redruth's Community Assessment and Treatment Unit (CATU) and the Same Day Emergency Care (SDEC).

Are you passionate about providing high quality patient care and would like to be part of this new development in the patient pathway?

Do you enjoy acute nursing and dynamic patient flow? The CATU and SDEC model is an innovative and dynamic patient pathway of which a key aspect is the assessment of the acute patient and treatment of symptoms aiming to return the patient to their home with the right support. You will be empowered to utilise a range of nursing skills (supported by training) to support patients through a clinical admission, assessment, and ongoing treatment.

You should possess commitment to team working and innovative delivery of excellent standards of care. You will need to be able to demonstrate a real enthusiasm for using extended skills and a desire for on-going professional development.

Main duties of the job

The CATU model is an innovative and dynamic patient pathway of which a key aspect is the assessment of the acute patient and treatment of symptoms aiming to return the patient to their home with the right support. You will be empowered to utilise a range of nursing skills to support patients through a clinical admission, assessment, and ongoing treatment.

The SDEC model is new for community hospitals. This model enables admission avoidance but provides patients with the essential assessments and treatment to enable them to return home the same day.

There will be opportunities to lead patient care, taking on differing nursing roles on the unit such as the day case referrals and ensuring that patient care is kept to a high standard. There will be a high amount of liaising with other services so communication skills are vital.

Over 4,000 people make up the Trust. This includes doctors, nurses, therapists, plus admin and support staff.

We work in people's homes, in community clinics and bases. Some staff work from one of our 13 community hospitals. Our aspiration is to have great people, provide great care, be a great place to work and a great partner.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.
