Community Children's Team of the Children's and Young People's division at North Middlesex Hospital is seeking to appoint a band 6 paediatric nurse as part of the new children's Hospital @ Home (H@H) service. The post is based at North Middlesex Hospital located in Edmonton, London and the service is commissioned to provide home based care to children in the borough's of Haringey and Enfield. We are a 7 day service, 365 days a year, working hours 07:30-20:00, consisting of early and late shifts.

The H@H pilot has been commissioned by NCL ICB. The function of the team will be to provide hospital standard care to children with acute nursing needs in the community. Working within a virtual ward model, the service will develop bespoke care pathways for children requiring nursing care which will avoid extended lengths of stay and admission. The team will work closely with Paediatricians and pharmacy colleagues to ensure a seamless transfer home and provide high quality nursing care according to their treatment plan.

An exciting opportunity has arisen for our newly establishment H@H service. This is a new service that will sit alongside an existing well established dynamic CCNT. The post holder will work within the multidisciplinary team to provide safe and appropriate management of children and young people with an acute illness which can be treated in the community. They will be working closely with the acute teams to ensure that patient safety is paramount.

They will be able to identify patients suitable for H@H and ensure a rapid discharge from ED or the ward. To plan, implement and evaluate their care. To review any re-admissions and identify potential reasons why this has occurred.

The H@H service supports the NHS Long Term Plan which emphasises the importance of care to be provided closer to home and aligns with the STP vision to improve urgent and emergency care for children. In turn, the H@H service will aim to support the 4 hour emergency department turnaround target and improve the flow of patients within North Middlesex.

We also provide community services and have a dedicated 0-19 service for children and young people in Enfield so that they can get the best possible start in life. This includes health visitors and school nurses who are delivering the national Healthy Child Programme, which provides a structured framework for the delivery of key interventions to support the health and wellbeing of children and families from 0 to 5 and school aged children from 5-19. The 0-19 service aims to improve pathways and partnerships with services in the hospital and deliver excellent care for the children and families.

We are seeking to appoint a dynamic and enthusiastic candidate to the role of Senior Staff Nurse for our newly created Paediatric Hospital@Home service to provide a high level service to all patients and staff within the department.

After several years of ever closer working, North Mid and Royal Free London are planning to come together as one organisation to enable them to go further and faster in improving services for patients and the health of the local community than they can achieve separately. The merger of North Mid into the RFL group is currently expected to take place on 1 January 2025, subject to NHS England and Secretary of State approval.
