Full Job Description

The range of assigned tasks will be carried out with the highest standard to support and enable qualified staff to provide patient care, which reflects the quality of standards, values and behaviours of Cwm Taf Morgannwg University Health Board (CTMUHB).

The post holder will be required to carry out a range of assigned tasks involving direct and indirect care for the benefit of the patient in accordance with agreed protocols.

The range of responsibilities will involve rotation as appropriate to the needs of the service and will include the provision of clinical sessions across a number of chronic conditions.

The post holder will be responsible for ensuring that all policies and procedures are observed, and meet the UHB requirements contained in the Health and Safety Policy.

Will also be expected to assist in development of policies relevant to speciality.

The post holder has the freedom to act within established parameters, work is managed rather than supervised. The post holder is required to act independently within UHB and professional guidelines, referring to supervisory management as appropriate.

Main duties of the job

Provide expert generalist care within the primary care setting, namely Public Health Nursing and First Contact nursing for the adults and children who reside within HMP & YOI Parc.

Provide specialist care for chronic disease management working autonomously in the prison premises to co-ordinate the care needs of patients.

Delegate duties to junior staff, as appropriate e.g. junior registered nurses, health care support workers, student nurses.

Be expected to support junior staff within their sphere of responsibility, including junior registered nurses, student nurses and health care support workers and contribute to the Continuing Professional Development of Practice Nursing and other Primary Care staff.

Deliver sensitive information during clinical consultations.

Discuss and facilitate the patients' decision-making process regarding future treatment plans.

Utilise motivational and negotiating techniques to encourage patients to follow the best course of action or treatment when they are resistant to change, frightened or in denial.
