Full Job Description

  • Please note that this role involves an equal amount of patient facing work which will involve travel to patient homes and online working and covers a 7 day period
  • As an autonomous practitioner with advancing skills to care for acutely unwell adults, the post holder will be responsible for the assessment and treatment of patients with complex health and social care needs. Ensuring provision of a holistic nursing assessment and high quality, safe, evidence-based care to patients and carers with the aim of maintaining patients in their own homes. This includes physical examination and detailed history taking, diagnosis of deterioration in pre-existing conditions and treatment planning. The post holder will work in this adaptable and evolving role, focusing on safe effective admission avoidance and supporting timely discharge from the acute and community hospitals, with a whole system approach. The post holder will co-ordinate care and strive to reduce the risk of acute re-admissions by identifying risks, implementing management plans, and collaborating with the whole community resource to facilitate ongoing care management.Main duties of the job Key Dimensions of the post are: o To be accountable to the Digital Health Ward manager when undertaking duties as a senior nurse o To work in an integrated way with all other agencies and other members of the multi-disciplinary team, across organisations. o To oversee and take charge of the Virtual Ward caseload and delegate to other team members. o To have an awareness of the need for the effective use of resources. o To be a safe, effective and responsible lone worker
