West Essex Community Health Services Respiratory Team have an exciting opportunity for a part time Senior Support Worker. This role will cover all of West Essex Localities. The role will require a good knowledge of Community Specialist Services working with the Senior Nursing Team to care for patients with Long Term Respiratory Conditions. The applicant will require excellent communication skills and general administration skills to ensure a high standard of record keeping via our EPR system Regrettably, we cannot offer sponsorship for all of our job roles. The post holder will work as part of the Community Respiratory Specialist Team which aims to provide ongoing care and monitoring for patients with Long Term Respiratory Conditions and prevent unnecessary admission to hospital. The role will include monitoring and supporting patients who attend community clinics and pulmonary rehabilitation programmes, visiting patients in their own homes, and providing telephone support and advice. The post holder will require good clinical skills appropriate for the role and will be an excellent communicator. Some administrative skills will be required, which will involve maintaining computer records and ensuring a high standard of record keeping., If you are joining our Trust in a fixed term role, please indicate on your New Starter Paperwork that you wish to join our staff bank.
- To work under supervision of a qualified nurse to assess, review, monitor and support patients with long term respiratory conditions according to evidence based guidelines.
- To be accountable for all aspects of own work and to manage the workload efficiently and effectively with regard to clinical priorities and use of time.
- To take a simple history of presenting complaint and progress.
- To give care as per individualised plan ensuring the highest possible standard.
- To assess effectiveness of nursing care.
- To provide evidence based therapeutic interventions as per individualised care plan.
- To document actions and findings in patients' notes in accordance with local and national standards.
- To report to the senior nursing staff.
- To participate in the Pulmonary Rehabilitation Programme as required.
- To be responsible for the safe and responsible use of equipment as per policy/ guidelines.
- To adhere to local and national infection control policy.
- To respond to clinical incidents in the appropriate manner according to organisational policy. The Mid and South Essex Integrated Care System (ICS) brings together key health, care, community, and voluntary sector organisations from across our area. Our aim is to work together in partnership to join up health and care services, improving our population's health and wellbeing and reducing health inequalities. One such example of this collaborative work is the launch of our HCA Academy, an ICS initiative between Essex Partnership University NHS Foundation Trust (EPUT) and Mid and South Essex NHS Foundation Trust (MSEFT) to support the incoming Health Care assistant (HCA) workforce. The academy models a 'one workforce' cross system approach to the onboarding, induction, education and experience of Healthcare Assistants. To support this 'one workforce model' the successful candidate for this post will be required to attend our 5 day (Monday to Friday) face to face Induction Programme at our MSE ICS Education Hub at Brentwood Community Hospital. Your start date will therefore fall on one of two of the HCA Academy induction dates across the month, which will be discussed and agreed during the recruitment process. Our HCA Academy Induction programme has been designed to train and support the successful candidate through the early stages of their employment at EPUT and will provide opportunities to enhance skills, knowledge and experience. Further information will be provided post interview. This will be followed by an additional 5 day (Monday to Friday) EPUT corporate / mandatory training welcome. (EPUT Welcome will be varied between face to face and virtual delivery).
