Full Job Description

Would you like to be instrumental in supporting a patient to avoid hospital admission and remain safe in their own homes?

Do you enjoy being responsive, thinking on your feet and fast problem solving?

If the answer is yes, a new role within the New Milton and Lymington therapy team may be of interest., To work as part of the community therapy team and take the lead in the Urgent Community Response for the team.

-To assess people at home who are at risk of hospital admission in next 24-48 hours due to a sudden and acute change in health condition such as infection or progression of palliative disease which results in sudden change in mobility. (N.B Patients may not have already had a medical assessment before seeing the therapist and therefore may need the clinician to recognise a deteriorating patient presentation and refer on for medical assessment when required).

-To establish a safe way to remain home if possible, or if not able to refer on to appropriate location to support their needs urgently to reduce likelihood of further injury/deterioration.

-Urgent Moving and handling assessment.

-Urgent positioning/pressure management.

-Urgent Falls interventions.

Other tasks:

-Referring on to other professionals within One team (District Nurses, Frailty Support Team), as well as GP, Charities, social services, and acute hospitals.

Applicants must be a UK Health Professions Council registered Occupational Therapist/Physiotherapist OR a Degree in Sports and Exercise Sciences / Sports Rehab or appropriate related field and be registered with the Society of Sports Therapists or BASRaT, and in possession of a full driving license and vehicle to be used for work. The post holder will also need to be willing to work flexibly to support our 7 day service model.
