Full Job Description

Sussex Community Foundation Trust has an exciting opportunity to recruit a Band 6 Occupational Therapist or Physiotherapist to the Hospital Rapid Discharge Team at Princess Royal Hospital, Haywards Heath. This role would suit a practitioner with highly developed leadership and people skills, who are able to balance clinical and non-clinical tasks within a fast paced, multi-disciplinary team with a clear vision for the future development of the service and staff. Candidates should be skilled in the assessment and treatment of patient's presenting within an A&E department., To comprehend the acuity of patients within the front door setting, understanding and facilitating complex hospital discharges ensuring patient flow. To complete patient assessments, consider clinical diagnosis and treatment of patients who may have acute complex and/or chronic presentation, under different specialities. To provide an effective high quality service to all patients and assist with the teaching to all members of the team including students. To contribute to service development within therapies and the wider MDT. To implement policy and policy changes in own clinical area and to assist with the setting and monitoring of standards of practice. To maintain close collaborative working between the multi-disciplinary team. To link day to day working with UHSx teams within the front door service.
