Full Job Description

We are looking for a Band 4 Senior Therapy Assistant to join our Community Neuro Rehab Team.
We provide short-term rehabilitation to enable patients, where possible, to remain in the community rather than be admitted to hospital or long-term care, and to live as independently as possible.
We work within patients' homes, rehabilitation groups, Community
Hospitals and other care settings, to ensure effective discharge planning and a seamless rehabilitation pathway. We provide holistic assessment of patients' needs and help patients achieve maximum independence in daily living activities and improved well-being.
Your role will be absolutely key in the support of our patients. You will help with the undertaking of treatment plans and use your clinical judgement to keep people as safe as possible. Full duties can be found in the job description.
Please note due to the community based nature, you will need a driving licence and a vehicle.
Additional Working Pattern: 7.5 hours each day Wednesday to Friday
