Full Job Description

The purpose of the Place team is to work with all NHS, Council and VCSE partners to deliver the local Place plans and support delivery of national NHS Operating Plan objectives for residents.

This is a broad portfolio including prevention, early diagnosis, primary care, integrated neighbourhood working, urgent and emergency care (including intermediate care), elective / planned care and cancer.

There will be a requirement to develop meaningful and collaborative working relationships with all local providers to deliver the ambitions and cultural change of the Place plans including implementation of the recommendation within the Fuller report (improved access, greater primary and secondary prevention, personalisation and effective management of long-term conditions).

Members of the Place team will liaise closely with all parts of the ICB 'core', including transformation, medical, quality and primary care to ensure there is consistent approach to place transformation.

The underpinning ethos is of partnership to tackle inequalities and improve health outcomes for all residents, using co-production, population health and quality improvement methodologies.

Main duties of the job

The post holder is responsible for the programme and project delivery of agreed objectives for Place, working closely with colleagues across LA, NHS and VCSE partners to deliver sustainable improvements that improve health outcomes and / or tackle health inequalities experienced byresidents.

The post-holder must be able to influence and engage across multiple stakeholders, building a virtual team across settings and organisational boundaries to deliver demonstrable improvements in residents' health, well-being and access to health services. Responsibilities will include working with members of the core ICB teams and local partners to ensure regular oversight of local variation within BLMK and with national standards applicable to all partners, This will include patient experience of access to services, update of screening and access to prevention services,such as health checks for people with LD and SMI and improved patient reported outcomes relating to end of life care and fall prevention services.As part of a multi-skilled team of change and improvement specialists, to lead the development and delivery of a number of complex transformational and other strategic projects that support the achievement of benefits and outcomes valued by, and agreed by, the organisation
andsystem.
