Senior (Type 1 & Pump) Diabetes Specialist Nurse / Dietitian

In partnership with members of the multi-disciplinary team the post holder will develop, implement and evaluate a seamless specialist service, ensuring that patients receive the highest standard of clinical care. They act as a specialist and will demonstrate a high level of expertise within the specialist service, providing specialist advice, education and support to healthcare professionals and patients. The post holder will actively undertake clinical audits and research within the specialist area.

To be a point of contact and clinical expert for patients, relatives and clinician colleagues in conjunction and with the full support of the clinical team. The post holder will be expected to ensure that the patient receives the highest possible standard of evidence-based care in line with current NICE/JBDS guidance., o To provide a world class service for patients with type 1 diabetes, including patients using MDI, CSII (insulin pump) therapy and/or continuous glucose monitoring (CGM - some using the Hybrid closed loop systems), and/or Flash technology. Involvement in pump starts and upgrades.o To help care for a cohort of patients (some complex) with excessive morbidity and mortality in an area with high health inequalities, language and cultural barriers and low healthy life expectancy. The post holder will be required to keep abreast of rapid advances in a complex technology, including CSII (insulin pumps), glucose sensor devices, integrated pump-sensor combinations, multiple software platforms that integrate these devices and their data downloads and patient-level support software including apps and online self-management tools. 1 & Pump patient support group.o The post holder may care for pregnant woman with type 1 diabetes who are using an insulin pump and / or (CGM). Optional.o The post holder will be an important member of the team and contact for both our Adults and Young Adults (alongside our YAD team).o As a registered nurse / dietitian you will be expected to consistently meet the performance standards as set out in the Code of Professional Conduct (NMC and HCPC).

We are looking for a dynamic forward thinking & motivated DSN or Dietitian to help develop and support the diabetes service for Barts Health. The post holder will be a team player with vision, drive and enthusiasm. The successful candidate will have demonstrable experience and proven clinical skills as a diabetes specialist nurse or dietitian.

The ideal candidate will have strong nurse/dietitian-led outpatient clinic experience in Type 1 services and either a working knowledge or a willingness to learn the new and developing diabetes technologies (insulin pumps; CGM and Hybrid closed loop).
