Full Job Description

The post holder will be responsible for the timely notification, and provision of closure (assurance) forms for Patient Safety Incidents, providing updates on progress with investigations and responding to queries.

To ensure the Health Board fulfils its reporting requirements to Regulatory bodies in relation to Patient Safety Incidents.

The post holder will be required to liaise directly with Welsh Government, NHS Executive, Health Inspectorate Wales and other regulatory bodies as required.

A key part of the role will be to track the progress of investigations against agreed timescales with senior staff seeking confirmation that agreed actions have been implemented to provide assurance to the Executive Team and Regulatory bodies.

The post holder will be a key member of the Organisational Learning Team for Putting Things Right team and work to support ongoing activities ensuring timely compliance with NHS Executive timescales.

The post holder is expected to work unsupervised using their own initiative and demonstrate appropriate analytical ability and comprehension skills. The post holder will display sensitivity, tact and diplomacy at all times and will liaise with staff across the organisation and outside stakeholders., Coordinate incident processes with professionals and external agencies. Initiate and organise plans which may be complex and multi-agency. Use appropriate databases to ensure all deliverable dates are tracked to meet NHS Executive compliance. Completion of Welsh Government Serious Incident and No Surprises notifications, Nationally Reportable Incidents (NRI) outcome forms, providing updates when needed and responding to WG request for further information. Meet local and national policy guidance and audit requirements re compliance with Welsh Government reporting of serious incidents.To maintain a central filing system (electronic) ensuring that all documents are filed correctly and there is effective version control management.Monitor and track action plans for Patient Safety Incidents to completion. Maintaining and updating RL Datix (incident reporting system) as an accurate database of Nationally Reportable Incidents reported to NHS Executive. Undertake specific delegated projects or
assignments formulating and adjusting plans as required.Liaising with Welsh Ambulance Safety Trust (WAST) to distribute and collate information for the joint investigation framework. Provide guidance to QPS teams with regards to triggering criterion. To support and steer through the Duty of Candour (DoC) process to completion.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
