Full Job Description

Our Single Point of Access (SPA) service is based at New Parks Health Centre. SPA provides a central point for all referrals, information, advice and signposting for Community Health Services.

We have full time and part time positions available for Service Coordinators to join our team. The post holder will deal with telephone calls, e-referrals, electronic faxes and emails coming into SPA and is responsible for the coordination and management of patients and professionals through defined pathways. The aim of this service is to ensure that our patients see the right person, at the right time and in the right place.,

  • Work as a team player, supporting other team members to be successful and deliver the best possible service from the Communication Centre.

  • Contribute to creating a culture of open, honest communication and feedback so that all team members benefit and learn from each other.

  • Receiving calls from professionals and patients dealing with queries and administrative tasks.

  • Organise and coordinate a range of activities between different organisations to prevent acute admission and to facilitate discharge in line with agreed protocols.

  • Making outbound calls to patients and professionals in order to coordinate patient care according to agreed pathways and service agreements.

  • Maintain your learning and compliance with training requirements for the role.

  • Participate in supervision via agreed review and appraisal mechanisms.

  • Ensure that data is kept secure and maintain the confidentiality of staff and patients at all times.

  • All records must be kept up to date and maintained in an accurate and diligent manner.

  • Contribute to the database for patients (SystmOne), updating records and contributing to the collection of data for audit purposes.

  • Contribute towards creating a high performance, continuous improvement environment by being proactive in spotting gaps in processes and bringing to the attention of your Manager.

  • Act upon patient and professional feedback in a timely and proactive manner, so that any service issues are addressed promptly., working patterns in line with our flexible working policy. For all substantive roles, new staff (excluding medical staff) are appointed subject to a 6-month probationary period (see Probation Policy). All jobs will require permission to work in the UK.
