Full Job Description

This is an exciting opportunity for an enthusiastic and highly organised individual to join the REACH Team as a Service Facilitator. As Service Facilitator, you will be working as part of a fast-paced team, supporting with the implementation of an electronic care planning system and undertaking day to day organisational and some administration duties. You will work alongside a variety of colleagues at all levels helping to contribute to the daily smooth running of the service. You will be maintaining and updating information using an electronic rostering and scheduling system and allocating referrals for packages of support to the most appropriate support teams, to help ensure a timely delivery of service.

We are looking for someone who can take a pro-active, flexible approach and is able to use their initiative and take responsibility for monitoring, maintaining, updating, and recording of data.

You should have a high level of organisational skills and the ability to respond appropriately to telephone and emails enquiries from internal and external stakeholders and have good interpersonal skills and the ability to work collaboratively with others.

Reablement and Community Home Support (REaCH) is a short-term service providing care to Stockport residents aged 18 years and over. REaCH provides a variety of services including Reablement, End of Life and Night services.
