Full Job Description

Provide operational leadership, line management and co-ordination across Urgent Community Response (UCR) Services, linking with system partners to provide effective and efficient service delivery.

Support the development of creative solutions across traditional organizational boundaries to deliver the right care, in the right place, at the right time by the right person.

Play a key role in embedding a collaborative culture, developing bespoke and creative solutions to address patient's needs and avoiding unnecessary hospital admissions.

Lead the promotion of the service to all potential referrers and stakeholders to ensure the service is accessed appropriately and having maximum impact, and to represent the team at appropriate engagement forums and meetings., To lead and manage UCR services, supporting operational issues on a day-to-day basis.

To be responsible for high quality service delivery and foster a culture of interdisciplinary, multi-agency working to deliver bespoke care solutions.

Develop positive and professional relationships across a wide range of internal and external groups, agencies, and service providers.

Provide and receive highly complex, sensitive, and potentially contentious information, including presenting information about UCR to a wide range of internal and external stakeholders in formal settings.

Understand the whole health & care system to establish priorities for the service and address these on a constant basis to ensure high quality outcomes for the patient and system. This will be highly complex information and require analysis, interpretation, and comparison of a range of options to establish the right course of action.

To manage and analyse service activity and performance data to demonstrate compliance with service targets and key performance indicators, identify trends and key areas for service development.
