Full Job Description

We are looking to recruit a full-time / fixed term (or secondment with internal line manager approval) Service Manager until December 2025 to cover maternity leave. The successful candidate will join the Countywide Hospital Discharge Management team and have responsibility for the Discharge Assessment and Review Team. The service manager will be responsible for management of the established team where the primary focus is around undertaking timely reviews following hospital discharge in line with the councils priorities.
The Service Manager will have the ability to establish and maintain close relationships with partner agencies and support the development of our offer to Derbyshire residents. You will use your relationships and skills in supporting efficient and creative solutions to support flow from the Acute Hospital Sites across our discharge pathways.
The Service Manager will embrace 'Discharge to assess' and 'Home First' approaches and be involved in necessary work to develop departmental and system wide responses that support individuals to have sustainable support in the community, avoiding long term institutional care.
Applicants should be able to demonstrate an excellent track record in their ability to support and manage others and develop professional autonomy and accountability. You will be responsible for the management of performance including delivering on outcomes identified within the council

The post is for a 5 from 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. Prior notice will be given in advance if this changes.

Saturday and Sunday hours will be paid at plain time where it is part of your normal working pattern unless it is between 7pm and 7am, where enhanced rates of pay apply. Enhanced pay rates are paid for Bank Holiday working.
