Full Job Description

The Single Point of Access provides a vital link 365 days of the year for referrals/information/advice and signposting for Adult Community Health Services including; Community Nursing, Community Therapy, Long Term Conditions, Phlebotomy, Continence, Integrated Community Palliative Care Service Stroke & Neurological Rehabilitation.

You will lead, line manage and motivate the team to deliver the desired results, through effective execution of duties. To provide day to day operational leadership and management responsibilities.

The role requires a continuous improvement focus, identifying root causes of issues and working with the rest of the leadership team to resolve them, as well as identifying and progressing opportunities to develop the service further. In addition, you will be required to manage the floor and handle referrals during busy times.

The work can be unpredictable and is demanding, excellent leadership and communication skills are called for due to the complex range of calls taken. The ability to prioritise, meet deadlines, organise a workload and be courteous at all times is paramount to this post.

The post will mainly be based in the SPA office at New Parks Health Centre, however circumstances may require you work in other areas of Community Health Services.

In order to meet the needs of the service the post requires you work on a rota in order to cover 7 days a week.,

  • Establishing effective leadership and communication.

  • Maintain your learning and compliance with training requirements for your role.

  • Participate in supervision via agreed review and appraisal mechanisms.

  • All records that the role is responsible for or modifies must be kept up to date and maintained in an accurate and diligent manner.

  • To work within and lead the SPA management team.

  • To work with health services, social services, primary care services, acute hospitals, community hospitals, GPs, EMAS, the voluntary and independent sector.

  • Communicate and present information effectively and appropriately.

  • Provide and receive complex and sensitive, sometimes contentious, information, using a motivational and persuasive management style to reassure and lead the team.

  • Liaise and work collaboratively with all members of the team and other agencies.

  • To adapt communication skills to meet the individual needs of service users/carers.

  • To attend relevant meetings and cascade information as appropriate., working patterns in line with our flexible working policy. For all substantive roles, new staff (excluding medical staff) are appointed subject to a 6-month probationary period (see Probation Policy). All jobs will require permission to work in the UK.
