Full Job Description

Responsible for the assessment of patient care needs and the development, implementation and evaluation of programmes of patient care. Co-ordinate the Nursing team to ensure the effective delivery of care. When delegated to do so; manage the ward in the absence of the Senior Sister / Unit Lead Nurse. Give effective specialist nursing care to the patients on the Cardiology ward. Demonstrate, supervise & lead learners in delivery of high quality Nursing & specialist care. Show commitment to the 6 Cs of Nursing and demonstrate exemplary Nursing practice and national standards for the Nursing, Assessment, Performance Framework( NAPF), Maintain accurate clinical observations of the patients condition invasively and/or non invasively Co-ordinate the implementation of a treatment plan for individuals diagnosed with coronary heart diseases and other conditions. Interpret the significance of clinical data e.g. through analysis of presenting signs and symptoms, 12 lead ECG performing and recognition, invasive monitoring (arterial and central), and adjusts / administers prescribed therapy accordingly. Able to effectively recognise peri-arrest / cardiac arrest situations using ABCDE approach and initiate appropriate therapy e.g. drugs. CPR, transcutaneous pacing. Work towards NAPF recommendations Act as a learning resource for pre and post registration staff (nursing, paramedical& rotators from Cath lab &CCU) and provide education about cardiology nursing, policies & procedures. Take an active role in daily ward rounds, advocating for patient and able to provide reasoned arguments. Maintain timely patient records by inputting episodes of admission, transfer & discharge, into CERNER & Miyaflow systems In the absence of the Ward Manager, receive 'messages' and takes appropriate action where possible, ensuring effective 'day to day' running of the unit. Demonstrate effective communication:-

  • daily as shift leader, with bed facilitators and managers,
  • when passing on messages, on behalf of the ward team,
  • as a link nurse, or when representing the Ward Manager in meetings., As the Junior Sister/ Charge Nurse you would be responsible for the day to day running of the ward. This involves taking responsibility for the safe staffing of the ward, managing sickness and absence, and coaching and developing junior staff., Coronary Care Unit is looking for a Junior Sister/ Charge Nurse who can uphold the ethos of the unit whilst ensuring continued development of both staff and service., Assessor of Nursing: -
  • Establish an individual's functional capabilities in the context of an emergency presentation. Establish the details of the individual needing care Maintain accurate clinical observations of the patients condition invasively and/or non invasively Contribute to assessing individuals with suspected coronary heart disease Take a presenting history from an individual to inform assessment of an individual Assess individuals' needs for psychological, emotional or social rehabilitation Conduct a pre-intervention assessment of an individual. Accurately complete all relevant documentations relating to patient assessment. Planner of Nursing: - Prioritise and refer individuals for further assessment and care Support individuals through the process of dying Develop, implement and review care plans with individuals Co-ordinate the implementation of a treatment plan for individuals diagnosed with coronary heart diseases and other conditions. Assess presenting conditions and determine the appropriate intervention for the individual Prioritise the interventions to be performed for an individual Effective early discharge planning. Practitioner of Nursing: - Give specialist care to patients and relatives: Administer drugs and treatments, including mixing IV additives for infusion or administering IV bolus as prescribed with safety, accuracy and intelligence within the Trust Policy. Assess patient care using the Trust's recognised assessment tools Interpret the significance of clinical data e.g. through analysis of presenting signs and symptoms, 12 lead ECG performing and recognition, invasive monitoring (arterial and central), and adjusts / administers prescribed therapy accordingly. Able to effectively recognise peri-arrest / cardiac arrest situations using ABCDE approach and initiate appropriate therapy e.g. drugs. CPR, transcutaneous pacing in line with the Trust drug policy and the UK Resuscitation Guidelines. Prepare & assist medical personnel and manage patients requiring transvenous pacing. Perform venepuncture and cannulation skills. Adheres to National standards for Infection control & prevention. Including completion of current audit tools. Lead by example and challenge non-adherence to policy. Prepare and manage unstable patients requiring escort out of trust for more specialised management. After required training; be able to transfer patients to other hospitals & departments with Intra-aortic-balloon-Pump in-situ. Access patients' clinical results via ICE, PACS, Horizon, medcon with up-to-date password to ensure timely interventions according to clinical need. Effectively add referrals to BVH onto white board to avoid un-necessary delays in interventional treatments. Communicate effectively with patients, their relatives and co-workers. Liaise with medical, paramedical, staff and other members of the MDT in the delivery of care. Take an active role in daily ward rounds, advocating for patient and able to provide reasoned arguments. Manage patient flow in accordance with patient-need and 4 hour waiting targets for the benefit of patients in the wider Trust. Demonstrate effective use of SBAR tool for patient handover in & out of the ward. Demonstrate, supervise & lead learners in delivery of high-quality Nursing & specialist care. Report clinical concerns to Ward Manager / Matron Evaluator of Nursing: - Assist with measuring the quality of nursing care. Measure goal achievements with patients, relatives and co-workers. Change care plans and goals according to evaluation. Work towards NAPF recommendations Learner/Researcher of Nursing: - Seek to develop new skills. Apply research findings to practice to ensure 'research based practice'. Teacher of nursing and health: - Assist with the identification of principles of nursing care given in the ward/department. Reflect on learning experiences with patients, relatives and their co-workers. Act as a key person in the promotion of a healthy lifestyle for patients and staff. Promote and delivers health education activities within the ward. Act as a learning resource for pre and post registration staff (nursing, paramedical& rotate from Cath lab &CCU) and provide education about cardiology nursing, policies & procedures. Manager of Nursing: - Assess work to be carried out and participate in workload assessment. Organise staff & assign duties according to competence & experience, time manage workload and organise appropriate equipment required for the completion of work. Supervise the delivery and quality of care given Challenge performance issues and participate in performance management. Lead by professional example. Praise and correct team where appropriate. Identify training needs through observation and discussion with team members. Report and receive reports of work activity. Take an active role in daily ward rounds, advocating for patient and able to provide reasoned arguments. Maintain timely patient records by inputting episodes of admission, transfer & discharge, into CERNER & Miyaflow systems Handle complaints by patients, members of the public and staff, in accordance with the trusts' policies. Assist the Ward Manager in the maintenance of safe systems of work, including Datix, Risk Register and Moving and Handling of patients. Conduct assigned staff appraisals in a timely manner, uploading to learning Hub. Ward Management Role: - In the absence of the Ward Manager, receive 'messages' and takes appropriate action where possible, ensuring effective 'day to day' running of the unit. Employ appropriate measures to ensure the ward has adequate staffing and skill mix e.g., in event of sickness. Ensure economical use of resources, stock rotation and ordering of deficient resources liaising with stock management team when necessary. Identify any deficiencies in the service provided, reporting to the ward managers where required. Liaise with patients, relatives and the MDT, providing up to date, research based information. Demonstrate effective communication:-

  • Professional person: - Abide by the legal requirements and statutory rules relating to practice e.g., NMC Code of Professional Conduct. Take responsibility for own personal development & education, including in-service training. Maintain standards of conduct and dress to sustain the public confidence, in accordance with the NMC Code of Professional Conduct and the Trusts policies. Show commitment to the 6 Cs of Nursing and demonstrate exemplary Nursing practice. Standards of Conduct: 1. Ensure that personal actions and conduct comply with the Trust Policies and Procedures e.g., Health and Safety, Fire Procedures, Tobacco Control Policy, Equal Opportunities, Confidentiality etc. 2. Complete all mandatory training in a timely manner e.g., Induction, CMT, Safeguarding, Moving & Handling, Fire safety, Equality & Diversity, Information Governance, Resuscitation, Conflict resolution, Blood Transfusion, and any other new mandatory training. 3.   • First Level Registration Qualification in specialty to diploma/degree level, or willingness to undertake study.
  • Cardiology Experience

  • Experience of recent employment in a pre-registration student nurse area.
