Full Job Description

The successful candidate will be required to carry out clinical assessments on those identified with a cognitive impairment. The candidate will work closely with the multidisciplinary teams on the wards to ensure person-centred care is being delivered. You will be required to review practice, provide specialist advice, support and make recommendations.

We are looking for a nurse or therapist with experience of working with people living with dementia, who is enthusiastic, innovative and forward thinking with a passion for improving the lives of people living with dementia.

The post holder will work with the dementia teams across the Trust to support the implementation and evaluation of a seamless Dementia care pathway, ensuring that patients receive the highest standard of clinical care, treatment and support throughout the hospital journey and beyond. They will specialise in the care of people with dementia and delirium and will demonstrate a willingness to develop expertise in the provision of specialist advice and support to ward teams, people with dementia and their carers. They will play a key role in the delivery of the trust Dementia education programme, providing training and evaluating training programmes. The post holder will participate in clinical audits and research within the specialist area.

Together with the dementia teams, the post holder will contribute to the embedding of KPI targets and NICE Quality standards across the Trust, and the implementation of objectives within local and National Dementia Strategies.

The post holder will influence the ongoing development of clinical practice within Dementia and delirium care practice, participate in research and contribute to the development of standards of care within the service, including the development of policies, protocols, guidelines and service developments in collaboration with multi- disciplinary colleagues.
